HEADLINE NURSING NEWS

 
TRANSPLANT DRUG RECALLED

The FDA announced the nationwide recall of SangCya, a generic version of cyclosporine available in liquid form. Neoral (brand name version) and capsules of cyclosporine are not part of the recall.

It appears as though taking SangCya with apple juice affects the absorption of the drug (by 20-30% according to research studies). The FDA has said that patients taking SangCya should continue their usual dose until they have consulted their physician. Since cyclosporine drug levels are routinely monitored, it is unlikely sub-therapeutic levels are occurring in these patients without the physician already being aware of the problem.


National Practitioner Data Bank (NPDB)
and the
Health Integrity and Protection Data Bank (HIPDB)

These are fairly new data bases that health care practitioners should be aware of, they are part of federal law and require that adverse actions against a practitioners license be reported to NPDM & HIPDB. Their purpose is really two fold:

  1. Improve health care quality

  2. Reduce costs of fraud and abuse in health care

While the information in the data base is not accessible to individual consumers, it is available to state boards and other health care entities, and you can do a self query ($10.00 fee).

IV IBUPROFEN FOR PREMIES

The New England Journal of Medicine released this report nearly two months early because of its potential value in treating patent ductus arteriosus. The study conducted by physicians in five Belgian ICNs compared treatment of ibuprofen to indomethacin in treatment of this common problem in premature infants. One-hundred and forty-eight patients were treated and were divided equally between each drug. While neither drug was clearly better at closing the duct that lets blood bypass the lungs (ibuprofen 52 (70%), indomethacin 49 (66%), ibuprofen does appear to cause less kidney dysfunction (5 babies) then that seen with indomethacin (14 babies).

Patent ductus arteriosus in normal in unborn babies and usually closes within one to two days after birth. However, this is often not the case with premature infants particularly those weighing in at two pounds or less.


MEDICARE RECIPIENTS MAY FINALLY GET
PRESCRIPTION COVERAGE

Senator Roth (R-DE) chairman of the Senate Finance Committee proposed a government-administered program to offer drug benefits to all Medicare recipients. Sen. Roth made his proposal at a closed meeting of the Senate Finance Committee last week. The move is unusual as this has always been a stalemate between the republicans and the democrats. Sen. Roth becomes a powerful ally for the democrats in bringing about this legislation.


FDA ADVISORY PANEL KEEPS CHOLESTEROL DRUGS
PRESCRIPTION ONLY

Twice this week an FDA panel voted to keep drugs that treat hypercholesterolemia from being sold over the counter. Pravachol (pravastatin), made by Bristol-Myers Squibb and, Mevacor (lovastatin), made by Merck & Co. will both still require a physicians order. The decision was based on the fact that patients with heart disease and diabetes cannot take these medications. The FDA is not bound by the panels decisions but it is unusual for them to not follow their recommendations.


INTERSTATE NURSE LICENSURE COMPACT

Basically the Interstate Nurse Licensure Compact allows nurses with a license in one member state to practice nursing in another member state. It also allows for provisions of reporting information between member states in the areas of nurse regulation, investigation and adverse actions. It also intends to promote compliance with the laws governing the practice of nursing in each state.

Arkansas, Delaware, Iowa, Maryland, North Carolina, Texas, Utah and Wisconsin have already put the compact into effect. Mississippi, Nebraska, and South Dekota have passed legislation which will take effect in 2001. In Maine, we are waiting for the SBON to make rules. In Idaho it has passed in the house of delegates and is in committee.


NEW YORK PASSES WHISTLEBLOWER PROTECTION

The New York State legislature passed a Healthcare Whistleblower Protection Act, which was introduced at the request of the New York State Nurses Association (NYSNA). The legislation is now waiting for the governor's signature. The new law will protect nurses and other healthcare workers from retaliation by employers. It should be considered every employees responsibility to report dangerous situations to appropriate regulatory agencies.


DRUGS & ADHD
Nurses Fight the Norm and Just Say No To Drugs

One of the greatest hypocrisies is our government's "Just Say No To Drugs" campaign while at the same time parents, educators and health care professionals are prescribing and administering record numbers of medications to "control" children with Attention Deficit Hyperactivity Disorder.

A group of schools nurses are fighting back. There are methods other than medications to help school aged children with ADHD and their web site Save One Starfish is an outstanding informational resource. They don't see ADHD as a disorder, but rather as a goal: "Achieving Dreams and Happiness without Drugs".

This multi-award winning site believes "the current medical fad seems to be the "pathologizing" of childhood. Educators and parents are becoming increasingly disturbed by symptoms commonly referred to as hyperactivity and inattention in the classroom. At the request of parents and teachers, doctors are diagnosing millions of children and labeling them disordered simply because they do not sit still in class and seem unable to attend to their lessons. Should we believe that millions of American children have brain disorders? Or should we look more closely at ourselves and to where our lifestyles have led us?" (from Save One Starfish site)

This is a non-commercial site devoted to treating children as the holistic human beings they are. While editorialized, this nurse writer thinks Save One Starfish is one of the best works of the nursing profession on the net to date. I applaud Mable, JB and BK for their excellent presentation of our profession to the Internet community and for their admirable work.

Great Fathers Create, Mold and Shape Great Nurses
A Tribute To Fathers

Great nurses are all around us. They do phenomenal things. They make powerful differences. Are you a great nurse who had an even greater father or grandfather or other male in your life who "great" fathered you? Did you or do you have a father who was like mine -- a father who challenged you to go beyond what the world expected of you? Who made statements like "I don't care what kind of job you end up in .. it could be digging ditches ... As long as they are straight ditches and the best ditches"! My father said that. He, also, made other statements to me that shaped my life and gave fodder to my own level -- such as it is for nurse greatness.

I believe that great nurses demonstrate this in a multiplicity of ways. They stand out in whatever they do. The dig great ditches of caring, sharing, counseling, supporting, encouraging and helping consistently. They understand what my fourth grade educated/graduated dad meant when he encouraged me and my six brothers and sisters to dig great ditches and to "get an education."

What great philosophy did your "dad" expound on about "greatness"? Did he remind you, like me, if you want to get ahead and make something out of yourself and get others to listen to you to get an education. Education can make, break, or promote a nurse. Are you honoring your "father figure" by using all you have learned or by continuing your quest for the right type of learning to boost you onward. Remember, we can get great learning anywhere -- from patients, clients, customers, friends, other nurses, peers, supervisors, doctors, housekeepers, janitors -- as well as, by sitting in a chair in inservice or at the local university. I would like to have you and me use the education we have gleaned from so many places to honor our "father" and/or "father figure".

Nursing Intervention:
Dealing With Patients Who Are Battered Women

How many times have you, personally, been told by a female patient, “Oh, I’m so clumsy! I ran into a door.” or “I slipped and fell. That’s how I broke my nose, teeth“, etc.? Did you believe her? Perhaps you thought that it seemed to be an inappropriate response, given the injuries sustained, but you thought it wasn’t your business. As nurses, both male and female, this IS our business.

I’d like to share something with you that is my personal experience, not as a nurse, but as a battered woman. If you’ve never been battered, it is a little difficult to understand why “she just doesn’t leave him”. If you have, then this is your story, too.

I lived on the south side of Chicago with a man named Robert. Robert was my last abuser out of many in a 13 year span of time. He turned out to be addicted, not to alcohol as I’d feared, but to diet pills (Didrex) and Tylenol #3. To read the rest of Michele's story, visit her site by clicking here.
